Below, I have tried to copy in my new book. In spite or because of using pdf, the formatting has been garbled, the marginal notes and all the illustrations left out. Nevertheless I hope it might pique the odd interest. Anyone interested in seeing the thing in its entirety should e-mail me directly - rather than 'posting a comment' - and I will send it to them as a pdf attachment.
